The fact it is a silent movie may be another obstacle to the comprehension of the story, which is a bit contorted.
Here’s the cast (as you can see Mana and Közi play 2 different roles):

THE PLOTThe movie begins showing a book tied by chains floating on the water. This book is found and opened by a mysterious man, who’s always hidden by a black coat (I don’t know who he is). He’ll appear many times in the movie, while always reading this book.
In the following scene Gackt’s hugging the blond girl, who’s the main character of the movie. She passed away and he embraces her dead body.
Then gackt awakens letting us understand that the previous scene was just a dream and a dark-haired girl is laying in the bed with him (he’s a playboy X°D I should rather say he loves doing stuff with women =_=) After he got up we can admire him while bare-chested (with some really long-waisted trousers) (Momiji: they almost arrive till his armpits °-°), until he get dressed and goes away riding a horse.
Our heroine (from now on I’m going to write “She/Her” with capital letters since they didn’t give Her a name, so that I don’t have to repeat “the heroine” over and over, considering She’s the only girl on the set) appears at the window of her palace and watches him going away, so She runs trying to reach him but She can’t do nothing but watching him departing sadly. Yuki appears beside Her and tries to comfort Her.
In the meanwhile Gackt has reached a tavern where he drinks till he gets drunk, maybe not to think about the nightmare he had just a while before. The tavern is owned by Mana who’s, without compromises, a pimp (here Gackt stocks up on girls..). Besides, that’s a feminine role in all respects. In the same tavern we meet again the dark man.
The scene changes again: Gackt has come back home; She’s having fun wearing different hats in front of the mirror while Gackt tells Her which of them fits Her better (She’s horrible anyway =_=) A maid comes in the room, she’s not well framed but we can recognize her: “she” is Mana!! And she looks really good! (that’s a feminine role, too) (Momiji: weird, isn’t it? X°D)
After a while, the two of them go out and while walking on the street, Gackt knock against the dark man. We can see now that he’s lame (but this is not important to the story). Kami appears for the first time in the movie (wonderful) observing the couple entering a subterranean tunnel which is “enlivened” by some cawing ravens.
The couple reach a fortune-teller, completely dressed in red as well as the make-up around his eyes: the face is hidden but recognizable everywhere! In fact we’re speaking about Közi ^_^ (I must say the red colour fits him).
He’s going to read the cards, but first he gives Gackt a dagger and he uses it to make a little cut on Her wrist and pours it in a bowl (she lost so much blood she should have been dead O_O they exagerated but... Let’s go on XD).
Közi put the cards on the table, while the couple watches. The last card he puts on the table is... The Death. So the cards are saying she’s going to die soon (it doesn’t matter X°D) (Momiji: well, there is a mistake because the Death doesn’t mean disgrace but change o.ò)(in fact, it would be a change for Gackt, from fiancee to single, it would be better since the way the girl looks X°°D)
She’s obviously devastated and spends Her time sighing, sitting in front of the window of Her room, staring into space. Even when Her maid Mana comes (this time we can see him clearly) She doesn’t change attitude and refuses the meals brought to Her. (Momiji: it’s not hard for me to imagine Mana’s hatred and murderous instincts, while constricted to serve someone even if he’s pretending XD) (right, usually the REST OF THE WORLD is serving Her Majesty the Count Mana X°°°D)
Some days later she seems to be in a good mood while assisting to a friendly duel between Gackt and Kami and won by Gackt (really???? X°°D) She and Yuki, who watched beside his sister and is happy to see Her in a good mood, congratulate to him but Kami is angry: he made a sorry figure in front of the girl he loves because of the man he hates, because Gackt is always unfaithful and Kami sees the fact like a double humiliation. (thanks to Cassy for these information ù_ù).
The scene changes again to show us for the third time the dark man thumbing through the pages of the book; then we see Her sleeping in Her bed and Yuki covering Her not to let Her be cold (what an adorable brother *O*) (XD)
The evening, Gackt, She, Yuki, Kami and Mana are watching an usher’s performance in the sitting room: the usher is Közi, and he’s really good in this role.
Days pass and She’s ill and forced in bed (eureka! XD) Yuki is devastated because he doesn’t know how to help Her, so he run outside and goes to the near town by horse.
The dark man appears for the fourth time and enters Mana’s tavern reading the book. Gackt is there too, he drinks as usual. Mana notices him and sensually caress his hand (XQ___) (Momiji: you got a diseased mind..ù_ù)
Suddenly Kami arrives, he stands in front of Gackt staring at him, then he pulls out a knife and cuts his wrist, exactly like Gackt had done with Her in front of the fortune-teller Közi. We can suppose then, that day he had followed them and assisted to the ritual.
Gackt understands that something must have happened and rushes out knocking against Yuki, who was searching for him just to warn him that Her health was getting worse.
They both hurry outside and Gackt takes Yuki’s horse to come back home, leaving the devastated friend there. Mana looks out of the door with a look on his face which seems to say “this time you did it, but next time you won’t resist me” to Gackt (or at least, in my opinion it’s like that X°D)
In the meanwhile Gackt has reached home, he goes inside, runs on the stairs, arrives in Her room and throw opens the door: She’s dead, Her arm is dangling on the bed. He comes closer walking like a zombie (sorry but I had to say it XD) and the same scenes as the one of the nightmare are shown: She and Gackt, on a meadow, hugging, she’s dead and he’s sad while embraces Her. I suppose than that gackt had a premonition.
When the movie ends we can enjoy Bel Air’s PV *O*
At the end of the PV we can watch the very final part of the movie: the dark man appears for the last time, on a bridge, the sealed book in his hands. Behind him we can see for an instant Gackt’s and Her souls (or at least I guess they are), the two souls walk one toward the other, they surpass, Gackt turns to watch Her but She doesn’t turn O_O
Then the dark man throws the book in the river below. The last scene shows the book while drowning more and more.
THE END
As you can see, the movie is not this great, besides it leaves us with many doubts and questions with no answer, first of all: Why did she die? We don't know..! And the most important...What on earth is the dark man there for??? O_O
Excluding these questions, we can say that a true Dear would do this and more than this for Gackt... In fact I watched the movie 3 times, not one... Three!!!
